What to expect

The following information may help alleviate some of your questions regarding your appointment with us.

Before your appointment

If your appointment is scheduled a few weeks in advance, we suggest you write down your questions and concerns so you can share them with our staff.

Please plan on arriving 15-30 minutes before your scheduled appointment time. This will allow time for you to register and complete any paperwork that is needed. If you are a new patient, you will be asked to complete a patient information form—including your address, phone and insurance information—prior to your appointment. You will also fill out a medical history form and read the privacy policy.

For your convenience, registration forms may be downloaded and completed before your arrival. The forms feature editable fields, which allow you to type your information directly in the form before printing.

What to bring

• Insurance card – please bring your card even if you download and complete the insurance section on the new patient information form
• Photo identification
• List of all medications
• Blood sugar records, as appropriate
• Blood glucose meter, as appropriate
